Appointment Information
All patients are seen by appointment only.
Appointments can be made between 8:30 a.m. and 5:00 p.m., Monday through Friday.
When calling for an appointment, please be prepared to provide the patient's name, address, phone numbers, birth date, type of insurance, reason for the appointment, and referring physician, if applicable.
Please remember to bring the following when you come for your appointment:
- Completed new patient paperwork (available online)
- Current insurance card(s) (we will need to make a copy for billing purposes along with a picture ID)
- Referral from your primary care physician if required by your insurance
- Medical records, x-ray or lab reports (if any) that address the reason for your visit
- List of current medications (name and dosage)
Out of consideration of other patients, please notify the office 24 hours in advance if you must cancel or reschedule your appointment. ****Please see our no show policy****
Also, keep in mind that you will need to provide a urine sample as part of your exam.
